Terms of Service

These Terms of Service ("Terms") govern your access to and use of the FormsSquare website, applications, and services (collectively, the "Service"), operated by Nabh Inc. ("Company," "we," "us," or "our"). By accessing or using the Service, you agree to be bound by these Terms. If you do not agree, do not use the Service.
1. Description of the Service
The Service provides tools that allow users to upload, digitize, analyze, organize, and manage insurance-related documents, including but not limited to cyber insurance application forms ("User Documents"). The Service may include automation, analytics, and artificial intelligence–assisted features designed to help users complete, review, and manage such documents. Nabh Inc. does not provide insurance, underwriting, legal, or regulatory advice.
2. Eligibility and Authority
You represent and warrant that you are at least 18 years old, you have the authority to enter into these Terms on behalf of yourself or the entity you represent, and your use of the Service complies with all applicable laws and regulations.
3. User Documents and Content Ownership
3.1 Ownership
You retain all right, title, and interest in and to your User Documents. Nabh Inc. does not claim ownership of User Documents.
3.2 License to Process
You grant Nabh Inc. a limited, non-exclusive, non-transferable license to host, store, process, analyze, and transform User Documents solely as necessary to provide the Service. This license terminates when you delete the User Documents or terminate your account, subject to reasonable technical and legal retention requirements.
4. User Representations and Responsibilities
You represent and warrant that you lawfully possess and have the right to upload and use all User Documents, your use of the Service does not infringe or violate the intellectual property rights of any third party, and you are solely responsible for the content, accuracy, and legality of User Documents. You agree not to upload documents you do not have the right to use.
5. No Form Library or Redistribution
The Service does not provide a shared library of insurance forms. Any documents uploaded to the Service are private to your account or organization, not accessible to other users, and not redistributed, published, or made available by Nabh Inc. Nabh Inc. does not sell, license, or distribute insurance carrier or MGA forms.
6. Artificial Intelligence Features
6.1 AI Processing
The Service may use automated and artificial intelligence–assisted technologies to extract data from User Documents, identify questions or fields, and assist with completion, analysis, or organization.
6.2 No Model Training on User Documents
Unless expressly agreed in writing, User Documents are not used to train global or shared AI models. Processing is limited to providing the Service to the user who uploaded the document.
6.3 AI Output Disclaimer
AI-generated outputs may be incomplete, inaccurate, or outdated. You remain solely responsible for reviewing and validating all outputs before use.
7. Acceptable Use
You agree not to use the Service for unlawful or fraudulent purposes, upload malicious code or content, reverse engineer or attempt to extract proprietary system logic, or use the Service to create or distribute unauthorized copies of third-party forms.
8. Intellectual Property
All intellectual property rights in the Service, including software, schemas, workflows, analytics, and interfaces, are owned by Nabh Inc. or its licensors. Nothing in these Terms grants you rights to Company intellectual property except as necessary to use the Service.
9. Confidentiality
Each party agrees to protect the confidentiality of non-public information received from the other party and to use such information only as necessary to perform under these Terms.
10. Termination
You may terminate your account at any time. Nabh Inc. may suspend or terminate access if you violate these Terms or applicable law. Upon termination, your access to the Service will cease and User Documents may be deleted in accordance with Company retention policies.
11. Disclaimers
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE." NABH INC. DISCLAIMS ALL WARRANTIES, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. NABH INC. DOES NOT WARRANT THAT THE SERVICE WILL BE ERROR-FREE OR UNINTERRUPTED.
12. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, NABH INC. SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR SPECIAL DAMAGES. NABH INC.'S TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT PAID BY YOU TO NABH INC. IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM.
13. Indemnification
You agree to indemnify and hold harmless Nabh Inc. from any claims, damages, or expenses arising from your User Documents, your violation of these Terms, or your infringement of third-party rights.
14. DMCA and Copyright Policy
Nabh Inc. respects intellectual property rights and responds to valid DMCA takedown notices. DMCA notices may be sent to info@nabh.com.
15. Governing Law
These Terms are governed by the laws of the State of California, without regard to conflict of law principles.
16. Changes to These Terms
Nabh Inc. may update these Terms from time to time. Continued use of the Service constitutes acceptance of the updated Terms.
